This also happened on an Amarr Commando. I was firing a couple swarms at a tank, ran a short distance, and tried to fire it again, but it wouldn't fire after the lock.

I've had this happen when I use a plasma cannon with a combat rifle and with a mass driver. What I do is fire my plasma cannon and switch to my second weapon as soon as I can and then hold R1 to fire, but my weapon doesn't fire until I press fire ...
