This is still happening at times. I switch logi suits at supply depot and 2 of my 3 equipment aren't on the item select wheel, the 1 that is there is in a different position. 1 and 2 missing, 3 is where 1 would normally be.

425k ISK is 2 matches, 3 at most. So after a max of 3 matches you can get either of the Heavy/Scout/Assault/Logistics to level 1.
