Seems to happen 10% of the time unless you have a real unlucky streak then it can happen multiple times in game and across multiple games. For me I seem to find at least 1 red heavy and an assault standing behind me when it happens.

Yep break the map into quadrants. Each team spawns into 2 different quadrants and never into the same quadrants. If you are in a squad then you always spawn with your squad.
