That's your answer right there.
And you're absolutely right, it can apply to any name and will be interpreted as needed. It's CCP's game, so CCP makes the rules. If you don't like it, then go play something else. I've been playing EVE and working with CCP's rules for nearly 9 years now, they've always been fair, but they expect you to be an adult. Vulgarity won't be tolerated and I appreciate that as a player. I don't want to see 'Flappy McScrotum' or any other such adolescent garbage when I'm playing, and if my nephew walks in the room I certainly don't want him seeing it either. |

Nope, not ok with that either, hence the 'walks in the room' I don't play the game in front of him, or any other child for that matter. When he comes in the room I pause the game or at least bring up the neocom, it gets me killed, but it limits his exposure...aside from the names scrolling on the right side of the screen.
And offhand references to bodyparts that everyone either has of will come into contact with isn't exactly what you did, you specifically referenced genitalia which in nearly all polite society is considered vulgar and inappropriate for general company. You want to talk about that stuff in a locker room with 'the boys' or in the privacy of your home that's your choice, but in mixed company it's just not cool. Not everyone is comfortable with it, so have some decency and be aware that other people may find it offensive. CCP's not the only company with such rules, so don't act like it was a surprise that you got your name revoked |

Perhaps you never noticed, but even with the neocom up the kills still scroll on the right side of the screen, so the names still popup and people can see them.
And yes, I'm from the US not that it makes much difference since I've played with EU folk on EVE for nearly a decade and I understand that things are a bit different and in all seriousness the names you've mentioned don't seriously offend me (i don't find them humorous either in case you're wondering), but I'm aware that they can offend other people so I wouldn't use them and it's not something I'd want my 4 year old nephew exposed to. I expect others to do the same, it's decent and polite which I understand is generally unheard of on the internet but that doesn't change my expectations or those of others.
As far as I'm aware CCP doesn't use any sort of automatic censor on EVE and I wouldn't expect them to be any different on Dust, but they are pretty active with letting people know they find something offensive and are usually pretty cool about letting people pick a new name via support ticket. The guidelines in the EULA are pretty clear that if CCP finds a name inappropriate they're going to change it. What's 'inappropriate' is up to CCP, and really, they're trying to put on a good face for Sony and the rest of the world with Dust. Letting players walk around with any of the names you've described, or anything similar, is just plain unprofessional.
