I find you bright eyed and bushy-tailed demeanour refreshing.
1.) That would certainly be an amazing thing for any game. 2.) Dust 514's players base is too small to sustain that kind of separation though i think it would be cool if we had a Test Server like EVE SiSi or whatever it is called. 3.) One of my corp members actually designed a full Unreal 3 map for Dust 514...... it was never used.

1) On the subject of a campaign mode, CCP once had plans to add PVE battles against rogue drones, but nothing ever came of it. In Fanfest (an annaul convention hosted by CCP for all their games) 2014, they announced that PVE would not be coming to Dust, but it is (supposedly) coming to Dust's successor -- Project Legion, a PC game that is essentially Dust 514 with better graphics, and some changes and features. 2) Matchmaking tries to put teams of equal skill against each other already, so I don't think the server thing is necessary, but the problem matchmaking faces is that there aren't enough players to fairly match players, so it ends up having to put ill-matched teams sometimes because it's the only way to make battles happen. Further server divisions wouldn't be a good idea given how low the player population is.
3) Map creator would be cool, but the dev team of Dust has been reduced significantly (hence the lack of new guns and stuff), so I don't think it's feasible. Another problem would be that maps created by players could be extremely imbalanced.
Yeah...... I'd make maps that basically favoured long range combat with massive open areas with minimal cover between heavily fortified objectives.
