Last night, I was in a match with Chances Ghost, Romulous Hex, etc. I gave them a grilling when they ordered their squad to back out of the train map. I was seriously p!ssed. I don't care if you don't want to play that map; when you back out of a match, that match becomes sh!t for everyone else. You are responsible for the 16v3's, the 14v7's, and the 15v6's.

You should have managed your time better to know when your PC's were. Your corp would have already made backup, and if it was you, you shouldn't have gone and tried to play pubs. Most PC corps even have backup for the backup. It comes down to how organied your corp is.
My friends often get on when I'm in a match, but it doesn't mean I should drop what I'm doing and leave the match - screwing over the rest of my squad and team - to go and pick them up. They can wait 10 minutes.
The third scenario you've descriped is primarily caused by people leaving the match in the first place. I don't blame you for leaving a match that's already lost, but if people stopped backing out in the beginning, you wouldn't be put in matches that have already started in the first place.

Who cares? I'm sure the people who are left to fight a 10v16 care. You should know that currently, Scotty does not refill pubs after people leave.
