As to your question, I am betting its til downtime, but I am only like 60 or 70% sure. I got banned once early on when RE damage to objectives were a thing, but I can't remember at all what the specifics were.

Every match should have a replay, so CCP can check if there's been any boosting/teamkilling and ban the bad guys.

Hard to avoid things that still get you FW band (someone who deliberately chooses to suffer through these contracts please confirm)
- Stupid/malicious bluedots shooting your RE/proxies standing inside the blast radius (or said explosives getting triggered by an enemy)
- Shooting a formerly friendly vehicle that was stolen by the enemy
- Orbital strike FF damage (WELP, A SINGLE BLUEDOT/MISSILE INSTALLATION/CRU IS STANDING NEXT TO 16 ENEMIES IN 7 TANKS, BETTER NOT STRIKE)
- Instantly roadkilling teammates who spawn on your vehicle's CRU
- Instantly roadkilling teammates who bail from your vehicle
