Yeah, the game is currently running at 720p, not much point going up to 1080p until they fix the texture loading issue (which I understand is a problem with the unreal engine, not the game per se).

Because CCP wants to avoid a power creep and turning everything into OHK super weapons and removing any meaningful choice. It is also much, much easier to nerf one weapon than to buff all of them.

I don't now what it is, but when it's happened to me it's always been with a sniper rifle, and very often against stationary targets. Re-zooming combined with a slight change in position seems to fix it with a bit of reliability.
