The biggest culprit in DS ramming is that shields take more damage than armor, so Gallente ships almost always survive vs Caldari ships, which is definitely not cool.
But again, this has been a thing for like 2 years. With virtually no resources being poured back into the game, problems really only get fixed if they effect the majority, and pilots are in no way the majority, nor have they ever been.

Was in a battle and the second a team mate spawned his DS a Grims just goes and lands on it... Didn't even give the poor man a chance to get off the ground... Back when I used to play it was basically my mantra to only spawn my DS directly under the MCC. Doesn't always save you from suicide DS's but it at least keeps you mostly safe from orbitals. You really gotta keep your eyes open, particularly for standard ships. Standard ships are pretty much used exclusively as elevators and disposable AV.

but seriously chances are you didn't see them because you weren't paying attention or got hit from behind, that or its an entirely different issue such as lag
there are time when I fire a rail gun or even a missile installation and the animation is delayed but damge is still dealt as normal I have also experienced the opposite of that but those issues are tied to lag Dude...invisible swarms have been a thing for a LOOONNNGGG time. Swarms have always had issues rendering, some times are worse than others but I don't think DUST has had a time where swarms would always render.
Any full time vehicle operator, particularly pilots, can tell you invisible swarms are very much a real thing. Unless they were fixed in that patch that never happened.
