More often than not, it's a result of high ping on my end compared to those I'm playing against. Being in a speed-tanked suit in a part of the game with slow framerate really helps too.
For instance, last night I was with some south american players on the EU servers late at night. There were a few encounters that the opponent had me dead to rights, but just couldn't kill me. I'd like to think I'm just awesome, but my tens of thousands of deaths in this game tell me otherwise. My opponents were players from quality corps (prima gallicus. game over, etc.) and I recognized their names as quality players, but I just strafed right through them, one after another after another -- and I'm sure they were cursing me the same way I curse Addict Punk or Kaziuka Sniper when I go up against them on the USA servers.
Low frame rate + fast moving suits + high ping = hit detection issues for your opponent....and that equation works both ways. Sometimes hit detection is due to terrain not rendering as well. Counter-snipers know this.

Counter-snipers know this. Primary reason why I fit my mk.0 logi sniper with a complex precision enhancer and complex range extension, it lets me see exactly where on the field everyone is. I'm fairly certain that only helps show people who are near you.
