The bug that prompts the player with a needle to a downed clone that isn't there is as frustrating a bug as any in the game. It has made me put away my needle for other equipment, even in ambush, I am sorry to say.
Half the time I risked my clone to rush to a locale to revive only to find nothing and getting my face melted was often my scouts reward.
I have rarely seen it mentioned but needs attention.
Oh and the sprint bug, and lag rubber band world, and nerf fatties with HMG's.
Chop Chop CCP. Everyone knows the forums run this MF'er.
Dominion over others is given not taken. Nothing has power over you but your own fear.

LOL I know this is besides the point but I just got say it because the topic reminded me of it.
You know what's crazy? When you see that needle gunning straight for you. You can see the little ants (reds) firing over your body but the medic is determined; the enemy is snickering. The medic has the grace of Neo, dodging bullets and running alongside walls to save his teammate who now lays underneath a blanket of constant projectiles.
Meanwhile I somehow forgot to adjust a fit before match and now I'm on the screen adjusting my fit with sweat on my forehead like I'm trying to disarm a bomb from the days of 80 movies "click the red switch, no the blue switch" plot device. I see the needle getting closer and my fingers are getting clumsier. I must hurry and respawn! Quickly!
And just when I go to hit X to complete-
*nanos injected zzzzziruum*
KILLED BY A GUY WHO IS NOW LAUGHING AT YOU
WTF! You stupid mother-
I tell you, a mission impossible theme wouldn't be odd in those situations.

I just hit the first uplink then OGǪ no chance of revive.
But not nearly as epic as your description.
