Keep in mind that this is a role-based game, so if you don't want people telling you you're not the right role to counter a heavy and to try to avoid them and let someone else on your team take care of them, then you haven't completely caught on to the reality of the game. There should still be plenty of players who aren't heavies, and worst case scenario you can use militia equipment like uplinks, nanohives, and repair tools to score some extra points and earn more ISK. Also, don't forget what are called "core skills," especially the armor and shield skills. These skills can increase your shields and armor by up to 25% without equipping any modules and make it easier to survive.
If you're an assault, try to get help from someone else on your team, and make sure you're not all attacking from the same angle. Flank him. Stay as far away from him as you can, because HMGs don't hit nearly as hard at a distance. Don't use mass drivers or plasma cannons, because Sentinels are resistant to splash damage. Use the longest-range rifle you have (such as rail rifles, scrambler rifles, or the tactical assault rifle). Also, if you have the ability to cook a flux grenade long enough that he won't be able to get out of its range, you can take a big chunk of his health away before you even start shooting. Most of the toughest Sentinels are armor-based, too, so if you can use weapons that are more effective against armor, like the combat rifle, rail rifle, or Magsec SMG, that will help a little bit.
If anyone on your team is running a scout, they can be very effective against heavies. Equip a shotgun and/or remote explosives, get behind the heavy (possibly using a cloak, but not necessarily), and either fire the shotgun straight into his head or drop the remote explosives at his feet, run away, and activate them. These are the tactics that have been hardest for me to avoid or counter; you have to be very aware of remote explosives because they don't have an icon like a grenade, and scouts can move so quickly that you often can't turn around enough to hit them before they've killed you.
I should also note that you never want to be running around solo in this game, as in never, at all, period, unless you're experienced and know exactly what you're doing. Even if you're not in a squad, just stay close to other blue dots and you improve your chances of survival, so long as you're also using your head.
