no map in particular, includes militia, and CBR17 (ADV normal) swarm launchers at least so I wouldn't only those exclusively, when you are out of the launchers render range (origin of shots fired) the swarms will not render when they enter the render distant up until they hit you or the max distance.
the 2nd bug is that after a merc has died his swarms will rise from his dead body anyways, this I`ve seen to many times and believe to be a bug. |

this happens all the time to me as well, very much so when flying a dropship where i may be flying in and out of the origins rendering range.
as for the dead rising rockets, not to defend the glitch, but it maybe happening due to the swarm launcher being more of a dead mans switch type of firing mode rather then a push to fire mode. |

Ive also had many bugs with swarm launchers, they seem to send a 2nd swarm when you destroy a target and they double fire in the same way at random times, these extra swarms do no damage but help snipers to zero in on your position much easier. |

I've had the invisible swarms problem. Like you said more than likely due to the current render range of players. Makes it real hard to figure out which direction you are being shot at from swarms when they are invisible.
And I have also had the swarms rising from dead bodies, but I figured the player fired them just as I killed them so didn't think much of it. |

This is because Uprising increased the graphics load and CCP/Shanghai cut the draw distance drastically to compensate.
Which means that Rail Guns Turrets cannot see 599m and swarms are most often invisible. There isn't even any mini-map-radar-thingie display to help figure out where the swarms are coming in from and the operator may or may not be visible.
I ran into a situation last evening (9 August, 2013) that I was hunting for the swarms. I could hear them, their explosions but couldn't tell if they were red or blue and all of this while I was actually infantry. A very irritating game mechanic. |

Also you have to get into cover early other wise if you get behind cover just before they hit you they will do a perfect stop/turn and come get you. I know this has been mentioned before but well nothing like keep bringing up the ever present glitches on hopes of getting them fixed one day. |
