This happens to me every other game. Unfortunately haven't taken any screenshots, but this is generally within the last 5 seconds of the game. 10 seconds seems to be enough time for it to register.

+1, I get this issue a lot also. It is not the same as models disappearing after the max render distance, but the errors occurring close to the maximum render range. It seems to be a LOD issue.
