1)Lock all actions while cloak is in effect such as:
-Climbing ladders
-Hacking
-Switching gear
And do not allow the cloak to be engaged when taking damage.
Until it is manually deactivated.
This would be the simplest most effective measure.

Have REs blow up as soon as they are thrown while cloaked only killing the user and don't allow shotguns to be equipped,and have the PG drain negatively effect the suits speed reducing it by 65%.

Cloak doesn't make you immune to bullets so if they're stupid enough to try to run and cloak in plain sight they'll die anyway

The cloak on a min scout already drains so much PG we can't use kin cats anyway. Not to mention speed is its only saving grace
With the widely abused permanent cloak bug being abused,all of the aforementioned in the OP are possible.

Lol,it wasn't fixed.
I've seen scouts still throwing REs while still cloaked.
And still using shotguns.
Today,Globfather (a guy i know) and i even encountered a C-1/Series sentinel that was invisible,we saw him shimmer when he'd move and he was using a forge gun.

There is a bug with a piece of equipment, so your solution is to reduce the overall effectiveness of the equipment.. rather than fix the bug  ... yeeeaahhh. Fix it first, see about changes later
This would fix the bug.
If you had to manually deactivate the cloak before you could switch or do any actions you couldn't possibly throw an RE could you?
The answer? N-O
Or how about use a shotgun?
The answer? N-O
This is a bug fix just not in a form you're willing to accept.

I don't need to be in the match to know that heavies being invisible (which would be a glitch/exploit) is not a reason to nerf clacks.

It's part of the forum culture now.
Well,the sentinel aside scouts can still shoot and use REs cloaked.
And i'm not really into "trends".
