That particular GMT is a trouble maker.
However, the real answer is the Squad Broke. This might have happened because your PS3/DUST Client has a network communication problem with CCP Servers. There seems to be some possible link to the number of Chats open. Try closing the ones you don't use often or at all.
I can run seven chats (below the Squad chat) and it is fine. Others have had to get it down to four. Your mileage will vary, unfortunately.
The other thing to check is that your router/firewall has the ports forwarded for CCP. The PS3 has several as well, with a specific few for voice chat. Since you sound like you have been running for awhile that may or may not be helpful. Once people have voice working that normally requires all the port issues to be resolved.
For the future try this sequence:
The Squad Leader (SL) should make Certain that everyone is in Quarters and not in any menus (Market, fittings, skills) Before Selecting the Battle Type.
When things are dicey (keep the Squad working) continue by SL saying when the display changes: Deployment, black screen, loading screen and that will let you know when someone is having a problem.
If the SL And another Squad Mate both try to Initiate Battle the Squad will normally break.
If the SL selects a Battle then cancels it but some or all of the Squad actually gets the Deployment screen then Squad might break.
As everyone gets used to the sequences most of that can be ignored unless you are breaking in a new Squad member.
Once the Squad is broken it must be rebuilt. Everyone can Leave Squad and the SL can as well. Then the SL can recreate the Squad and re-invite everyone. If someone Cannot Leave then that person will need to restart DUST. I have never had to reset the PS3 unless DUST actually Froze (hard crash).
Note that anyone that is having some lag issues or has been playing for hours and hours and hours it might be best to have them restart DUST as well. This is simpler when everyone is doing it versus them lagging out very next Battle.
