I happened twice in a row and I had to reset the game in order to clear it up.

For some reason the client is partially disconnected. That is teh best I can call it. From the players POV the client continue to look as if it is connected, but it is no longer able to update or receive updates from the server causing the timeout and missing team roster and ISK/SP updates, and may also be responsible for the failed removal from the squad the player were in. The partial disconnect also happens if you leave the client alone for too long. I assume that you are using a different socket/stream during battles, as the problem rarely show up till after a battle is completed, as seen with the player still being in the squad, failure to manually leave the squad, failure to receive updates from the server regarding skill points applied, isk used or received etc.

If it's just because that particular game mode is too busy, then changing the squad around is just allowing time for a spot for a group to open up. If there are other problems like the ones ZeHealingHurts mentioned, then it's probably the player that's bugged.

The PS3 game client thinks it's fully connected, but it is not receiving responses from the server when it makes requests, such as enter a battle. After I get these, nothing works.

Anytime i change an option it can take a long time for the "saving.." message to stop appearing on my screen. This seems to be related to network connectedness.

I basically take this message to mean the game timed out while looking for a server which could mean all the servers are simply full. Usually if I wait a couple minutes and try again it clears. It use to be much worse when Codex first launched, now it's just a rare annoyance.
