UE-- 5% fees on EVERYTHING you sell in/of/from the game... oh AND a developer fee for running the program up to date. Mind that Unity's dev fee is higher, but UE's sales fee lasts as long as your program. That's not really how this works. CCP doubtless paid either a lump sum or ongoing licensing fee to license their use of UE3/4. For businesses this is the MO for buying software. Unreal subsequently released Unreal Engine 4 under a broader license which allows anyone to use it essentially for free unless you start making money off of something developed under that license, at which point you've agreed that they'll get 5% of sales.
