1. Activate voice chat in options
2. Create a squad
3. Go on the squad tab and it should say,"Activate voice" and click on that.
4. Do the same thing on corp tab (activate voice)
5. TRY to deactivate voice (notice how it doesnt deactivate?)
6. Turn off voice chat in options
7. "You have been disconnected from DUST due to an internal error"
8. Ta-da!
Goodbye, world!

Moving to Technical Support/Bugs.
Also, can you try uninstalling and reinstalling the client? I've had reports that this issue has been fixed when players do that.

1. Open "Technical Support/Bugs" forum section.
2. Start a new topic.
3. Paste these steps there.
Seriously, bugs get more attention there than here and reproduction steps are critical so they'd love to have them.

Hi, thank you for contacting us.
I have few questions about these steps. Does the first step mean turning the voice chat from enabled to disabled in options? Is the GÇ£Deactivate voiceGÇ¥ in the third steps happening just after creating a squad? Of course, a video is the best way to make these steps clear.

Delamorui, do the programmers think this is related to this problem:
-- Channel shows voice is on(green dot), but name listing in channel does not (no speech bubble with speaker symbol inside.)
-- Voice does not move channels nor deactivate, though all the appropriate menu options do appear.
-- Mic symbol appears when I press-to-talk, but apparently my voice is not going through. (Mic-checks are not responded to.)
-- I do hear others in channel.
