I don't like blank applications so you should include some basic details. Example as shown
Minmatar Light, Plasma Cannon, MLT/STD/ADV/PRO as necessary, chosen role.
Our timezone is UK DST  so I am interested in recruiting a rep for US and Asian timezones to assist me in gauging combat skills of members.
You don't have to provide info as shown but in a later situation you won't be allowed to join Planetary Conquest battles.

I'm not sure about joining a Corporation with few members
I find larger corporations to be a little too demanding of their members with far too many Directors. The Doomslaying Democratic League is designed only to be a small corporation, this means people know each other better and this shows in the battlefield.
Why don't you have any districts
Planetary Conquest isn't easy stuff - first of all you need solid income to the Mercenary Wallet and a large and diverse player base. Both of which are difficult to maintain. I'm sure one day we can come to the strength to take on PC but for now we won't touch PC.
Tax Rate?
Now, most corporation will charge 10% or maybe 20%. That is a lot to put aside. We will offer a low tax rate of 5% and this will rise and fall during the fluctuation of the player base. If you recruit one member, I'll refund your next 2 taxes up to 60,000 ISK.
Donations
I don't mind that some people don't donate. This is not an issue while I remain CEO. 
Board of Directors, Wassat?
This Board comprises of all the Directors in our Corporation + CEO (Me). We deal with justice and potential kicks and bans.
